Job Description

Intensive Care Coordinator

Would you like to help ensure youth and families in the Weber County and Box Elder County communities are receiving the proper support to lead safe and healthy lives?

Do you have experience using the High Fidelity Wraparound Model? If yes, then this role as the Intensive Care Coordinator is the role for you!

PRINCIPAL DUTIES

The Intensive Care Coordinator will :

  • Facilitate High Fidelity Wraparound Model services for families : utilize a strengths-based approach to safety planning, development of family team and family support systems, and wraparound planning for the purpose of maintaining children in their homes, schools and communities by utilizing the 10 Principles of High Fidelity Wraparound : Family Voice & Choice, Team-Based, Community Based, Collaboration, Strengths Based, Individualized, Outcomes Based, Culturally Competent, Unconditional
  • Develop wraparound plan with families and agency partners with measurable outcomes and time bound specific objectives;

develop & maintain a crisis prevention plan throughout the process with each youth / family

  • Coordinate provider services and ensure the quality of services for youth and families
  • Oversee the achievement of goals to ensure needs are met and family vision is at the forefront; observe progress of the family with a goal for transition within 6 months
  • Balance perspective of family, professionals and community partners to come up with the best outcomes
  • Maintain a caseload of 10 (rural) or 12 families at all times
  • Prepare detailed case documentation including : opening and closing referral records and completing assessments, Strengths Needs & Cultural Discoveries, Wraparound Plans, contact notes, safety and crisis prevention plans, and correspondence, etc.
  • Provide regular communication and close collaboration with multiple community partners (e.g. Juvenile Court, Division of Juvenile Justice Services, Division of Services for People with Disabilities and Local Mental Health and Substance Use Disorder Treatment Facilities)

PREFERENCES

Preference may be given to candidates with the following skills or experience :

  • Bachelor's Degree in any relevant field or equivalent experience in any human services related field providing direct services to families OR 4+ years of working experience in in a relevant field, such as public health, behavioral health, etc.
  • Active Social Service Worker license in the state of Utah

Health Insurance - Public Employee Health Program (PEHP) :

  • 3 medical plan options. Choose a Traditional or High Deductible Health Plan. There is an Opt-Out Cash Benefit option for those that have other medical insurance coverage.
  • 5 dental plan options, with an Opt-Out Cash Benefit option for those that have other dental insurance coverage.
  • 4 vision plan options under EyeMed or OptiCare

Retirement - Utah Retirement Systems (URS) :

  • Employees who have any service credit with an employer covered by the Utah Retirement Systems prior to 7 / 1 / 2011 are in the Tier I Public Employees System.
  • Employees hired on or after 7 / 1 / 2011 and who have never been in the URS retirement system have two options for retirement in the Tier II System.

Tier 2 Hybrid Retirement System. Employer contributes an amount equal to 10% of your salary between both plans (pension and 401(k))Tier 2 Defined Contribution Plan.

Employer contributes an amount equal to 10% of your salary to a 401(k) plan only.

Employer contributions to your 401(k) are vested after four years.

Paid Time Off :

  • Annual leave and sick leave are accrued at 4 hours per pay period each (4 hours annual, 4 hours sick).
  • Annual leave amounts increase with years of service.
  • We have 12 paid holidays.

Supplemental Information

Conditions of Employment :

  • Some travel throughout Weber County and Box Elder County will be required for this position.
  • The candidate selected for this position will either be given access to a state-owned vehicle, or will receive mileage reimbursement for use of a personal vehicle.
  • Due to this position requiring travel via motor vehicle, candidates will be required to have a valid Driver License.
  • Continued employment is contingent upon the successful completion of a fingerprinting background check.

Work requires physical exertion. May require the ability to stand; walk over rough surfaces; bend, crouch, stoop, stretch, reach, lift moderately heavy items (up to 50 lbs.
