Summary
Provides behavioral health services to people with substance use disorder. Screens and assesses clients to determine the need for substance use treatment.
Provides referrals to community-based organizations and coordinates care as needed. Responsible for facilitating therapeutic group and individual counseling sessions.
Responsible for completing and maintaining detailed OASAS-compliant case records.
Responsibilities
- Conduct intake interviews, biopsychosocial assessments, mental health and other screenings.
- Formulate treatment plans and treatment plan reviews and schedule participants for services.
- Provide individual and group counseling services using evidence-based practices.
- Act as participant’s case manager, coordinating care within Crossroads and provide referrals to community-based resources (healthcare, social services, and administrative e.
g. help obtaining insurance).
- Complete accurate and timely documentation in the electronic health record system.
- Collect samples for toxicology tests.
- Prepare monthly updates for court or other referring agencies.
- Participate in treatment team case reviews.
- Develop discharge / transfer plans.
- Participate in staff meetings, case conferences, supervisory meetings, and trainings.
- Identify and report opportunities to improve the quality of care, staff effectiveness, and participant satisfaction.
- Additional responsibilities as assigned.
Qualifications
- Credentialed Alcohol and Substance Abuse Counselor (CASAC) or CASAC-T or LMSW
- Minimum of two years’ substance use counseling and case management experience
- Commitment to and experience in working with people from diverse cultural backgrounds, people with substance use disorder, and people who are involved, or at risk of involvement, with the legal system.
- Strong interpersonal and relationship-building skills
- Computer literacy (word processing, e-mail, internet research, data entry).
- Excellent clinical writing skills, including the ability to type clinical notes in the electronic health record system contemporaneously during sessions.
- Bilingual (English / Spanish) preferred.

Schedule
Mon- Fri from 7am-3pm
Salary range will be dependent upon credentials / certifications and experience level.
