As a practitioner and educator, you will leverageyour expertise as an NP or PA to provide transitional and chronic care,mitigating hospital readmissions and emergency room visits.

Responsibilities
Conduct thorough examinations of patients and review theirmedical records

Inform patients about their health conditions and prescribedmedications
