About The Position
Job Title : Registered Nurse (RN) - Hospice Job Specialty : Hospice Care Job Duration : 14 Weeks Shift : Monday to Friday, 8 a.
m. to 5 p.m., with potential call rotation every 3-4 weeks Guaranteed Hours : 40 per week Experience : Minimum of one (1) year experience as an RN in acute care hospital, oncology, long-term care, hospice, or home health.
School internships and clinical hours may be considered as experience in individual cases. License : Current license to practice nursing in the state of employment Certifications : BLS (American Heart Association), Current automobile insurance, and valid driver's license Must-Have : - Knowledge of terminally ill patients and their families, along with an understanding of hospice care- Excellent patient assessment skills- Ability to effectively operate nursing assessment tools, computers, and standard office equipment- Good oral and written communication skills- Knowledge of Medicare / Medicaid guidelines Job Description : Provide direct patient care services to hospice patients and supportive care to their families- Manage an interdisciplinary group consisting of Licensed Practical / Vocational Nurses, Certified Nursing Assistants, and support staff to enhance quality care- Perform initial and ongoing patient assessments and manage clinical records- Develop and implement Hospice Plans of Care in conjunction with physicians and interdisciplinary team members- Monitor patient conditions, report changes, and document care accurately- Ensure compliance with infection control measures and standard precautions- Participate in Quality Assessment and Performance Improvement Program- Attend staff meetings and complete required compliance training- Maintain up-to-date knowledge of pain and symptom management in terminally ill patients- Provide education to patients, families, and caregivers on care needs identified in the Plan of Care- Schedule and monitor aide services and perform supervisory visits- Act as primary liaison between physician, patient / family, and hospice team- Participate in agency on-call schedule as requiredJoin our team to provide compassionate care and support to hospice patients and their families, ensuring the highest quality of care.
