Benefits offered for Per Diem / Part Time Team Members :

Pay Range (depending on experience) :

NVA : $40 / hr

Re-visit : $40

Evals : $85

Schedule / Shift : PRN / Per Diem as needed, weekdays only

Territory / Location : Gilbert / Mesa / Chandler and surrounding areas

Responsibilities

• Conduct psychosocial assessments of patients to understand their needs within the context of their illness and environment.
• Develop and implement tailored intervention plans based on assessment findings.
• Maintain accurate clinical records for all patients referred to social work services.
• Offer information and referral services to patients, families, and caregivers regarding practical and environmental needs.
• Serve as a liaison between patients, families, caregivers, and community agencies to facilitate access to support services.
