Primary Care Physician - Internal Medicine or Family Medicine opportunity in Houston.
Excellent opportunity for a TX licensed, BC / BE Primary Care Physician (Internal Medicine or Family Medicine) for this growing value-based medicine practice in Houston, TX.
This fast-growing, senior focused, team approach to primary care provides a positive and well-balanced environment. This model is positioned to provide better outcomes for senior patients as we provide multi-disciplinary services and coordinated care supported by community relationships.

Requirements :
- Board Certified or Board Eligible Internal Medicine, Family Medicine or Geriatrician
- Active and unrestricted DEA
- Knowledge of HEDIS scores helpful
- Bilingual Spanish Speaking
