About The Position

Job Title : Skilled Nursing Physical Therapist Job Specialty : Therapy Physical Therapy Job Duration : 13 Weeks Shift : 8 : 00 AM - 4 : 30 PM Guaranteed Hours : 40 hours per week Experience : Open to new graduates License : Must be licensed in the state(s) of practice as a Physical Therapist Certifications : Appropriate education level required in accordance with state licensure Must-Have : Valid driver's license may be required Job Description : Plan, organize, and deliver physical therapy programs to help clients / patients recover and improve their mobility, relieve pain, increase strength, and prevent deformities.

• Conduct examinations, evaluations, and interventions for patients affected by injury or disease.- Utilize tools and techniques to administer physical therapy interventions while following safeguards.
• Document client / patient information for evaluation and establish therapeutic plans, modifying them as necessary.- Educate clients / patients and their families about treatment plans / schedules and at-home exercises.
• Apply knowledge of age-related treatments and evaluations suitable for the needs of clients / patients.- Incorporate current research into the daily treatment of clients / patients.
• Focus primarily on intervention in post-acute, long-term care, and home health care environments.- Demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements.
• Exhibit flexibility with variable work schedules / assignments.- Interact with individuals outside of the department, including providers, case managers, third-party payers, public organizations, and contractual partners.
• Supervise assistants, aides, and students as they provide patient care, treatment, or other services.- Provide ongoing education and training to peers in areas of expertise.
• Identify common patient safety risks within the organization and propose strategies for prevention.- Analyze and record the therapeutic progress of clients / patients;

recommend necessary adjustments.- Apply physical therapy tools and technologies according to a patient’s individual case.

• Conduct each step of the physical therapy process effectively.- Demonstrate ability to educate diverse audiences on patient health.
• Direct the implementation of the physical therapy process in various service settings.- Evaluate and modify physical therapy plans based on a client / patient’s health status.
• Ensure client / patient safety during treatment (e.g., while using equipment).- Monitor patient responses and reactions during therapy.
• Adhere to legal regulations and organizational policies when discussing patient health information.- Seek opportunities to serve as a mentor and understand the value of self-motivation.
• Recognize ethical responsibilities within areas of specialty.- Follow all guidelines, regulations, standards, and safety procedures in the therapy process.
