Danny's Home Healthcare is a family owned and operated organization, with extensive personal experience in the home healthcare industry! The Home Health Medical Concierge position is a wonderful opportunity to grow and excel in a caring and family-like environment! The right candidate for this position should have sales background and have or obtain relationships within the medical community.

This is a field marketing position, seeking someone who lives in Santa Barbara and willing to drive and cover Santa Maria.

The Home Health Medical Concierge identifies, develops, and maintains relationships between the agency and hospitals, nursing homes, physicians and community organizations that result in the acquisition of patient referrals for the agency.

Essential Expectations and Procedural Responsibilities :

  • coordinate services for patients discharged from hospitals and other referral sources.
  • obtain valid Medicare and other referrals as outlined in offer letter.
  • identify patients PCP and obtain authorization for home health services.
  • obtain doctor signatures for various home care orders and document accordingly.
  • assist in development of patient care plan and coordinate home care needs.
  • other duties as assigned by supervisor

Must have a valid driver's license.

Must pass a background check.
