Part Time Nurse - RN Co-Responder Team

The Co-Responder Program is closely linked to the Triage Program and the Crisis Residential services. These programs are designed to provide an immediate response to individuals experiencing a psychiatric crisis.

Job Requirements :

• A current Illinois RN or LPN license
• Adequate written skills to accurately complete required documentation within the time frames prescribed
• Current CPR certification or able to obtain within 3 months

Essential Responsibilities :

• Demonstrates essential nursing skills; responds to medical crises in the community
• Assesses physical / medical needs of Co-Responder
• Coordinates medical appointments for Co-Responder clients and provides transportation and support when needed
• Completes Medication Administration and Training for Co-Responder clients
• Completes prior authorizations as needed for clients medications
• Maintains communication with team leader and coordinator on medication assistance requests and budgets
• Builds relationships with medical community and providers and advocates for Co-Responder clients to get appropriate care
• Provides education to Co-Responder clients regarding medical problems and prevention issues
• Provides education on nutrition, wellness, medications and symptom management to Co-Responder clients either group or individual sessions
• Other responsibilities as assigned.
