Responsibilities :
- Evaluate patients, create care plans, and communicate effectively with patients, families, and the care team.
- Ensure every patient receives quality care and serve as a driver of the care team.
Qualifications :
- Graduate of an accredited school of nursing with a current state license as a registered nurse.
- 2+ years of nursing experience in a clinical care setting (hospice experience preferred).
- Reliable transportation and ability to effectively lift at least 25 lbs.
