The Connections Care Coordinator is responsible for maintaining and monitoring the memory care (Connections) holistic wellness program, including but not restricted to : assisting the Director of Engagement with input into resident assessments, leading neuroplasticity programming (Connect-Move-Learn cycles),, muscular strength, aerobic and flexibility training, wellness promotion, social-recreational activities and outings and volunteer program development and community resource coordination.

Highlights of the Connections Care Coordinator Position :

• High school diploma or equivalent
• Minimum 1 year experience activities or recreation service experience
• Certified Dementia Practitioner or eligible for certification.
• Willingness to work in a hands-on role with residents and staff.
• Must be able to drive a company vehicle (town car and 14 passenger van)
• Must possess a current driver's license and ability to meet approved driver requirements

The salary range for this position is $16.28 to $19.00. Pay is based on several factors including but not limited to education, work experience, certifications, etc.
