Our client is seeking a full-time RN Case Manager for Home Health in Brookpark, OH. The ideal candidate will provide skilled nursing care, demonstrate effective communication and collaboration, and ensure patient safety.
This role requires strong organizational skills, the ability to work independently, and a commitment to maintaining confidentiality.
Experience in home health care is essential. Coverage areas include Parma, Middleburg, Brooklyn, Brookpark, Fairview Park, Westpark, and surrounding areas.
Qualifications
Required :
- Associate degree or higher
- 1+ years of experience in any of the following :
- Hospice
- Acute Care
- Emergency Department (ED)
- Critical Care
- Burn Unit / Wound Clinic
- Oncology
- Home Health Care
- Medical-Surgical (Med-Surg)
- Valid Registered Nurse (RN) license in Ohio (OH)
- Valid American Heart Association Basic Life Support (AHA BLS)
Preferred :
- Bilingual in Spanish
- RN License Number
Key Responsibilities
- Provide direct or indirect skilled comprehensive nursing care to a variety of age and diagnostic groups
- Utilize principles of the nursing process for data collection, planning, and evaluation of nursing care
- Implement prevention, therapeutic, and rehabilitative nursing care
- Use effective interviewing techniques; set limits, and collect data based on specific role
- Communicate and collaborate effectively with healthcare providers, third parties, patients, and families
- Maintain awareness of patients' comfort and safety needs
- Observe patients, record significant conditions and reactions, and notify supervisor or physician of patients' conditions and reactions to drugs, treatments, and significant incidents
- Respond to life-saving situations based on nursing standards, policies, procedures, and protocols
- Use oral and written communications to convey pertinent information to members of the healthcare team in a timely manner

- Equipment Provided : Tablet, Phone
Schedule and Shift Details
- 8 : 30 AM - 4 : 30 PM, 1 weekend per month
- Includes working one summer and one winter holiday
