job_description.job_card.job_descriptionMinimum Expectation : 5 visits or 10 hours per week Visit Pay : $100 - $125 per visit Competitive, Weekly pay with mileage, travel time, and cell phone reimbursement! Medical for full-time, dental, vision and supplemental benefits available for everyone. 1 : 1 patient ratio; where you make a difference in our patients care with flexible assignments to fit your needs! Continuing education benefits and discounted courses through Colorado Christian University and Rasmussen University Company field employees who are paid mileage or have a leased vehicle are eligible for a retention bonus plan during two bonus periods. Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, COTA, PTA and SLP, focused on the patient’s plan of care and goals Observe movements, conduct assessments, document progress and modify care plan as needed Assist patient with exercises and hands-on therapy to manage pain and increase mobility Educate patients and family on their plan of care, therapy goals, home exercises, and self-care Graduate of an accredited Physical Therapist Program and active PT license in Colorado Minimum of three (3) years of physical therapy experience, ideally one (1) year in home healthcare CPR Certification (demonstration course required) Knowledge of state and federal home health regulations Good clinical judgment, strong interpersonal skills, excellent time management and compassion
