Providence at Home is a growing division of Providence Life Services , which provides a continuum of healthcare, focused on senior living.
Providence Hospice is seeking a Hospice Social Worker, Part Time in Tinley Park, IL . We are looking for a compassionate Social Worker to join our established, experienced hospice team.
We provide exceptional patient centered care to patients and families facing end of life.
Schedule for position : Must be able to work (3) days per week 8 : 00AM-4 : 30PM.
Join our team and make a difference by providing, dignity, comfort and peace to patients in their final days.
Summary of essential job functions :
- Evaluate patient, family needs and support them through the end stages of life and to assure that the medically related emotional and social needs of the resident are met / maintained on an individual basis.
- Ordering Durable Medical Equipment and planning discharges from hospitals and / or nursing homes.
- Visiting patients and families on a weekly, bi-monthly, or monthly basis to provide ongoing counseling and support as patients are transitioning and dealing with end of life issues.
Requirements
- Masters degree in Social Work.
- Licensed Social Worker in the State of Illinois.
- Minimum of 1 year Hospice experience preferred.
