Provides general medical and psychiatric nursing services, medical screenings, obtains laboratory specimens, administration, and delivery of opioid medications (methadone or buprenorphine), consultation and education, and other intraagency functions.

Reports to the OTP Nurse Coordinator or their delegate.

Requires completing several competing work priorities in a time-restricted environment. Works with a challenging population of patients, often from diverse backgrounds, with varying degrees of life stress and co-morbid general

medical and psychiatric conditions. Requires work on weekdays, weekends, and holidays. Some evening hours may be required, when necessary, to provide coverage to the Buprenorphine Clinic or to facilitate the evening educational Medical Aspects Group.

The work schedule is based on agency needs. In emergency situations, employees may be required to work additional hours in preparation for severe weather conditions such as predicted snowstorms or hurricanes.

The Program Supervisor may also approve for an employee to make a home visit to deliver opioid medications to a patient that has a condition that makes it difficult to come to the clinic (e.

g., post-op recovery) and the physician believes the patient cannot properly use take-home medication. On occasion, the nurse may medicate a patient who is too ill to come into the clinic and must remain in their vehicle outside the entrance to the Clinic.

PHYSICAL REQUIREMENTS OF THE JOB : While performing the duties of this job, the

employee may be required to stand for extended periods of time, utilize a computer, and operate

various types of office equipment : copier, printer, facsimile machine. May require the lifting of

boxes weighing up to thirty pounds.

ESSENTIAL DUTIES AND EXPECTATIONS :

1. Performs general and psychiatric nursing services, including but not limited to, medication

administration and delivery, intramuscular injections, weights, vital signs, body mass indices,

glucose monitoring, mental status examinations, and phlebotomy.

2. Administers and delivers medications in accordance with physician orders and documents

activities in accordance with OTP and other NCSB policies and procedures.

3. On a daily basis, verifies opening quantity of methadone and buprenorphine stock; verifies and

documents closing quantities of methadone and buprenorphine stock.

4. Maintains daily chain of custody of take-home bottles of methadone, ensuring accurate

delivery, storage and return of undelivered bottles back to the pharmacist in accordance with

program policy and procedures.

5. Provides clinical surveillance for indication of recent alcohol or other substance use that may

require the withholding of methadone or buprenorphine and consultation with a program

physician.

6. Ensures follow-up of abnormal laboratory results and the referral of patients to the appropriate

physician for the medical evaluation of any patient physical complaints.

7. Collects drug screen specimens (monitored and unmonitored) as required by patient's

treatment plan.

8. Screens all female patients for pregnancy at enrollment and as necessary; reports positive

pregnancy test results to both the program physician and the patient's care coordinator to assure

linkage to appropriate services.

9. Provides nursing services within the Opioid Treatment Program and the Buprenorphine Clinic

including initial and ongoing screenings, admission nursing assessments, specimen collection,

medication administration, referral and consultation, scheduling appointments, and other

administrative activities in accordance with the QTP and the Buprenorphine Clinic policies and

procedures. Documents all activities according to agency requirements.

10. Maintains up to date, well documented, and legible medical records that comply with Federal

and State requirements, program requirements, and accreditation standards (CARF.)

11. Facilitates individual and group education to patients.

12. Completes pre-treatment nursing assessments, to include medical and psychiatric histories, to

assist in determining appropriateness of the patient for admission. The OTP LPN does not make

the determination of appropriateness for admission but collects and shares the information with

the program's Registered Nurse and physician for final determination.

13. Assists the physician with patient evaluations, admissions, and interventions.

14. Performs phlebotomy services on patients to collect necessary laboratory specimens.

15. Participates in case reviews as a member of the inter-disciplinary team.

CSB Staff roles may include assisting other units at times to assist them with coverage needs,

specialized assistance, or to cross-train. Their category of employment is indicated below with

Not Staff hired into administrative positions (not direct service) can work in units other than

their primary role to provide administrative activities within their skill set, education, and

training. They may not work in units as a provider of direct services. They may serve in

voluntary" activities outside of the clinical service arena of the CSB's core services working

with the public for community outreach events, service fairs, homeless outreach activities,

homeless day shelters, neighborhood outreach events, trainings, etc. as long as their role is in an

assistance, trainer, or support function, not a clinical role.

Yes 1 Staff hired into direct service positions (direct service, monitoring direct service,

supervising direct service) can work in units other than their primary role to provide

administrative or direct service activities within their skill set, education, and training. They may

also serve in "voluntary" activities outside of the CSB's core services working with the public

for community outreach events, service fairs, homeless outreach activities, homeless day

shelters, neighborhood outreach events, trainings, etc. in a direct service, clinical, or

administrative role.

Yes Staff who are responsible for providing assistance in emergency situations will be

assigned to a role that is appropriate to their comfort level, skills, education, and experience.

PERFORMANCE REQUIREMENTS :

Knowledge :

  • General Nursing skills.
  • Effective interventions with individuals who have opioid dependency, with or without cooccurring

psychiatric disorders.

  • Client Rights Regulations.
  • Familiarity of clinical functioning of Opioid Treatment programs and services.
  • State licensure requirements.
  • Current pharmacotherapy protocols for psychiatric disorders, including medications, side

effects, dosages, and routes of administration.

Diagnostic criteria included in the current Diagnostic and Statistical Manual of Mental

Disorders; signs and symptoms of mental disorders, including co-occurring disorders, etc.

Skills :

  • Linking, coordinating, monitoring, assessing.
  • Crisis prevention, intervention, and management.
  • Familiarity with collecting specimens for drug testing (urine and saliva based.)
  • Familiarity with the use of an alcohol breath analyzer.
  • Phlebotomy skills for obtaining blood for lab specimens.
  • Maintenance of client records in accordance with all regulatory standards.
  • Liaison with community resources.
  • Advocacy.
  • Boundary and limit setting.
  • Facilitate group and individual educational or skill building sessions.
  • Administer medications, including the use of a computerized dispensing system.

EDUCATIONAL & LICENSURE REQUIREMENTS :

Licensure as a Licensed Practical Nurse from the Virginia State Board of Nursing or license

from a participating state in the Nursing Licensure Compact (NLC). One-year nursing

experience in a medical environment performing patient care duties preferred with one-year prior

experience working in an Opioid Replacement treatment facility, but not required. Experience

with persons seeking treatment for substance use disorders, with or without co-occurring mental

disorders and experience in the administration of opioid medication is desirable.

Possession of a valid driver's license and acceptable driving record as reported by the

Department of Motor Vehicles are required for persons using a personal or agency vehicle in the

performance of their duties.
