POSITION SUMMARY :
It is the function of the speech pathologist, through physician referrals to evaluate and provide service to homebound patients of the agency who have speech and language problems.
ESSENTIAL FUNCTIONS : Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties :
- Administers speech therapy evaluations when requested by the agency for patients who are accepted for care only by the agency in accordance with its admission policies.
- Informs the patient / caregiver of his / her rights as per agency policy. Includes patient / caregiver in initial care planning and when modifying plan of care.
- Observes, records and reports to the multidisciplinary team the patients reaction to treatment and any changes in the patients condition.
- Provides direct age-appropriate speech service to patients under a physicians plan of care. Records and reports to the physician and the nurse case manager patients response to therapy.
- Consults with the multidisciplinary team and integrates speech therapy services into the total patient plan of care.
- Provides formal and informal patient education based on age-appropriate educational goals mutually set by patient / caregiver and members of the health care team.
- Observes and evaluates the performance of those assisting in the speech program and provides guidance as necessary to assure a high quality of service.
- Supervises the home health aide when appropriate.
- Counsels the patients and their family members regarding the patient's speech and language disturbances.
- Reevaluates the patient's progress and therapy program making changes as necessary and under the physician's direction.
- Assists in referring patients to speech and audiology clinics and other community agencies.
- Submits all documentation of patient care according to Agency standards.
- Through individual and group staff education programs, prepares the nursing and other staff to assist patients with their speech and language problems.
- Maintains appropriate records on visits to patients, including evaluations and plans for care of speech patients.
- Provides liaison contact and follow-up with other community agencies.
- Represents the agency at community functions as indicated.
- Follows all agency policies and procedures.

JOB REQUIREMENTS
MINIMUM REQUIRED QUALIFICATIONS :
- Possesses a certificate of clinical competence in speech of the American Speech and Hearing Association.
- Speech pathology licensure in the Commonwealth of Pennsylvania.
- Current CPR certification.
- Current driver's license.
