Next Care Hospice is a fast growing hospice agency that provides compassionate care to our patients and their families during the end-of-life journey.
We are committed in providing continuous training to our staff with growth opportunities across the clinical and administrative tracks.
We are currently in need of an family nurse practitioner in the Lynwood area to assist with in home visits and rounding at local hospitals.
One year minimum experience is preferred. The APRN must have experience in hospice or palliative care or be willing to learn at an accelerated pace.
You must also be able to bring each visit to completion with recording of the notes in the EMR system. Bilingual is preferred.
We offer competitive hourly rates for rounding as well as per visit rate for in-home visits.
You will be responsible for the assessment, diagnosis, and referral orders for the assigned patients.
The ideal candidate will be a responsible and well-trained professional able to give the best care to a variety of patients and their families You will be compassionate and cool-tempered.
You will be able to follow health and safety guidelines faithfully and consistently. You will work cooperatively with the attending and / or rounding physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team.
Must be able to utilize computers and be comfortable with EMR (Hospice MD). Hospice MD training can be provided.
QUALIFICATIONS :
Current NP license within the State.
Hospice experience required
Knowledge of EMR software
