Position Overview :
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our Assisted Nursing Facility in New York.
The LPN will provide high-quality care to elderly residents, supporting their medical and personal needs while working closely with registered nurses (RNs) and physicians.
This role is crucial in maintaining the health, dignity, and well-being of our residents in compliance with New York State regulations.
Key Responsibilities :
- Provide personalized daily care to residents, including assistance with bathing, dressing, feeding, and mobility.
- Monitor residents’ vital signs (e.g., blood pressure, temperature, pulse, respiratory rate) and report any changes in their health status to the nursing team.
- Administer medications as prescribed, perform wound care, and assist with other clinical procedures under the supervision of RNs and physicians.
- Maintain accurate resident medical records, documenting care, treatments, observations, and any changes in condition.
- Collaborate with the healthcare team to develop, implement, and evaluate individualized care plans for each resident.
- Offer emotional support and education to residents and their families, explaining medications, treatments, and any ongoing care requirements.
- Ensure a clean, safe, and home-like environment, adhering to infection control, safety, and health standards as required by New York State regulations.
- Respond quickly and effectively to emergency situations, following established protocols to ensure resident safety and well-being.
- Participate in resident care meetings and contribute to discussions aimed at improving care outcomes.
- Support residents' independence and dignity by fostering a respectful and compassionate care environment.
Qualifications :
- Active and valid LPN license in the State of New York
- Completion of an accredited LPN program
- Basic Life Support (BLS) certification
- Strong understanding of elderly care, including common geriatric conditions
- Knowledge of New York State long-term care regulations and nursing practices
- Excellent communication and interpersonal skills, with the ability to build positive relationships with residents, families, and team members
- Compassionate and attentive to the needs of the elderly population
- Ability to work both independently and as part of a collaborative healthcare team
- Familiarity with electronic medical records (EMR) systems
