Description
The Home Care LVN will assess patient's needs, plan definite interventions and provide direct patient care as directed by the RN Case Manager.
The LVN will communicate patient status to appropriate team members and physicians.
Required Education : Current Texas LVN License
Required Licenses / Certifications :
Current American Heart Association CPR Certificate, Valid Driver’s License and Car Insurance
Required Experience : Minimal 1 year Hospital Nursing
Preferred Experience :
1-3 years Medical Surgical Nursing
