About SDAIHC :

We're currently undergoing exciting new expansions, including a $35 million campaign to construct a modern community health center and a partnership with the County of San Diego's Refugee Health Assessment Program to address the health concerns of newly arrived refugees.

Summary :

The LVN will provide nursing care and support under the supervision of a registered nurse (RN) or provider in the clinic.

The LVN will assist with patient care, including examinations, treatments, and procedures, while ensuring compliance with FQHC standards, state regulations, and healthcare policies.

Key Responsibilities :

Patient Care :

• Perform patient intake, including vital signs, medical history, and health assessments.
• Administer medications, injections, and vaccinations as prescribed by the physician or nurse practitioner.
• Assist with routine diagnostic tests (e.g., EKGs, lab draws) and clinical procedures.
• Provide basic wound care and other nursing services within the LVN's scope of practice.
• Assist with patient education on preventive care, chronic disease management, and medication usage.

Clinical Support :

• Collaborate with healthcare providers to implement patient care plans.
• Document patient interactions and care in the Electronic Health Record (EHR) in compliance with FQHC and regulatory requirements.
• Ensure exam rooms and equipment are cleaned and maintained according to infection control protocols.
• Monitor patient flow and work with the clinic team to ensure efficient scheduling and minimize patient wait times.

Care Coordination :

• Coordinate care with other healthcare professionals, including referrals to specialists and follow-up appointments.
• Assist in managing chronic care conditions, including diabetes, hypertension, and asthma, by following FQHC guidelines and care protocols.
• Support patients in accessing community resources and services as needed.

Compliance & Reporting :

• Ensure compliance with FQHC regulations, The Joint Commission (JCAHO) standards, and state and federal healthcare guidelines.
• Report patient concerns or changes in condition to the supervising RN or physician promptly.
• Participate in quality improvement initiatives and ensure adherence to FQHC performance measures.

Skills and Competencies :

• Strong interpersonal and communication skills to work effectively with patients and the healthcare team.
• Ability to manage multiple tasks and prioritize in a fast-paced environment.
• Sensitivity to and understanding of diverse populations, including underserved communities.
• Proficient with Microsoft Office Suite or related software.
• Proficiency with or the ability to quickly learn the organizations EMR and other billing software systems.

Qualifications :

• Current LVN license in the state of employment.
• Minimum of 1-2 years of clinical nursing experience.

Preferred :

• Knowledge of FQHC and community health practices
• Experience with Electronic Health Records (EHR) systems (e.g., ECw).

Special Conditions of Employment :

• Current BLS CPR certification with American Heart Association.
• Has a clean driving record and insurance as required by the state.
• Has reliable transportation.

Physical and Mental Requirements :

• Able to lift / move up to 20 pounds, move from place to place.
• Able to stand, bend and reach for prolonged periods.
• Ability to do math, organize and prioritize workload, work effectively and efficiently under stress.
• Ability to supervise, multitask, understand, and follow instructions.
• Ability to proficiently read, write, speak, and understand English.

• Uses all appropriate equipment and / or tools to ensure workplace safety.
• Immediately reports unsafe working conditions.

• Maintains privacy and security of all patients, employee, and volunteer information and access to such information. Such information is accessed on a need-to-know basis for business purposes only.

Flexibility : Available for all shifts and, when required, able to work evenings and weekends
