Together these facilities offer patients thebest rehab and nursing care they can obtain after their hospitaldischarge.

• Licensed Practical Nurse LPN willprovide daily support to the unit leadership & respondappropriately / timely to resident / family needs within the LPN scopeof practice
• LPN will use nursing processjudgment & skills to set priorities necessary to deliverquality direct resident care & make decisions promoting thesafety / quality direct care toresidents
• Licensed Practical Nurse LPN willperform / administer standard nursing treatments and / or medications& observes resident for desired or unwantedeffects
• Communicate & collaborate withappropriate member(s) within the care team to revise / update theNursing Care Plan or any pertinent findings including lab / testresults incidents & / or concerns
• LicensedPractical Nurse LPN will monitor & document vitals treatmentsadministered & residents responses
• Prepareorganize oversee monitor & evaluate delivery of care ofresident CNA assignments & activities considering the categoryof needs & competency of personnel

Requirements :

• Graduate of an approved school ofNursing
• Licensed & currently registered ingood standing in New York as an LPN
• Dedicatedto providing extraordinary clinically sound & compassionatecare
• Must possess a spirit of cooperationcaring & enthusiasm to create an atmosphere conducive torehabilitation & optimal residenthealth
• Genuinely interested in helping illinjured disabled elderly emotionally upset & sometimeschallenging residents
• Have excellentcommunication customer service & organizationskills
• Work well under pressure & willingto work harmoniously with all staff
