The Home Health Patient Care Manager is responsiblefor the overall supervision and coordination of clinical services.Coordinates and supervises an interdisciplinary team of staff toassure the continuity of high-quality care to home health patientsassigned to the team's area in accordance with physician prescribedplan of care, and all applicable state and federal laws andregulations.
- Receives referrals andensures appropriate clinician and / or therapist(s) assignments fortimely patient evaluation by signing off after authorization andplotting start of care (SOC) visits.
- Coordinates determination of patient home healthbenefits, medical necessity, and ongoing insuranceapprovals.
- Ensures patient needs arecontinually assessed and care rendered is individualized to patientneeds, appropriate and reasonable, meets home health eligibilitycriteria, and is in accordance with physician orders.
- Oversees and assures development, implementation, andupdates to the individualized patient plan of care, asappropriate.
- Manages and documents phone callsfrom physicians, clinicians, patients, referral sources, andcommunicates patient updates / new orders to clinicians.
Usescoordination notes to document, as needed andappropriate.
- Reviews assessments and plans ofcare daily, per assigned workflow, and consults clinicians withrecommendations, as appropriate.
- Coordinatesall aspects of care with all disciplines, physicians, durablemedical equipment providers, caregivers / family members,transferring facilities, and any other applicable healthcareproviders.
- Follows-up on lab and otherclinical diagnostic test, physician contact, and significantchanges in the patient condition to ensure adequate physiciannotification, follow-up, and needed plan of care modifications andcommunicates such to clinicians.
- Schedules,prepares for, facilitates, and documents case conference / SOCreports and facilitates effective exchange of information acrossdisciplines especially with adverse findings, changes in patientcondition, daily and urgent updates, as necessary.
- Assists clinicians in coordinating the transfer anddischarge of patients from agency services as indicated by thephysician.
- Receives report from fieldclinicians prior to scheduled days off on patient status andongoing needs.
- Processes new orders andupdates the visit frequency, as appropriate, when the on-call RNtakes supplemental verbal orders which alter frequency goingforward.
- Writes and processes orders whentaking verbal orders directly from the physician and communicatessuch to field clinicians.
- Assures payer changedocumentation is completed properly and timely, asrequired.
- Reviews clinician visit notes weeklyto ensure timely, complete, appropriate, and accurate submission ofall documentation by field staff.
Takes necessary action to correctadverse findings and communicates trending to clinicaldirector
- Reviews, evaluates, and supervisesservice delivery to ensure appropriateness of care and utilizationof services, equipment, and supplies through activities such asrandom patient visits, medical record reviews and caseconferences.
- Enters infections andincidents / occurrences into the online Risk Management IncidentReporting System, as specified by policy.
- Assists in the orientation of new agencypersonnel.
- Provides direction and leadershipto clinical team members in collaboration with the clinicaldirector.
- Provides direct patient care, asnecessary, in accordance to scope of practice and physicianorders.
- Participates in QAPIprogram.
- Assures compliance with and ensurestimely follow up on daily clinical and coding edits.
- Directs clinicians in utilizing best practiceinterventions when finalizing Plan of Care for allpatients.
- Participates in on-callrotation.
- Follows-up with On-Call eventsdaily.
- Receives report from weekend andafter-hours clinicians admitting new patients.
- Completes LHC required learning courses, additionalassignments per Executive Director request, as well as any statespecific required training per state regulation / practice actrequirements.
- Directs team in adherence to andparticipates in the Episode Management process.
- All other duties as assigned.
Education & Experience
- Current RN licensure in state of practice
- Current CPR certification required
- Current Driver's License, vehicle insurance, and accessto a dependable vehicle orpublic transportation
