Job Description Summary

The Patient Family Counselor (PFC) will assess and treat patients with physical disability due to traumatic injuries or illnesses to assist patients in dealing with emotional reaction to their disability.

The PFC obtains patient case history and conducts initial psychological assessment. As clinically indicated, the PFC provides psychotherapy services for patients, families, and within groups.

The PFC additionally provides behavioral management plans and suicide assessments, as needed. The PFC acts as an integral part of the treatment team for their designated inpatient floor.

Job Description

The Patient Family Counselor will :

Interviews patients, families and other appropriate personnel to obtain information regarding patient's personal, medical, psychiatric, substance use, and social history.

Provide consultation and therapeutic intervention for individuals, families, caregivers, and groups. Engage in evidenced-based interventions, including, but not limited to Cognitive-Behavioral Therapy (CBT), Trauma-Focused CBT (TF-CBT), Acceptance and Commitment Therapy (ACT), Dialectic Behavioral Therapy (DBT), Mindfulness based interventions, and relaxation training.

Lead and / or cofacilitate psychoeducational groups.

Perform social evaluations including, family dynamics, caregiving abilities, communication patterns, high risk for suicide, neglect or abuse, and plans based on evaluation findings.

Provide crisis intervention when necessary.

Educate interdisciplinary group on special needs related to the cultural differences, learning style, learning limitations, communication style, and roles within the family.

Consults with treatment teams regarding psychological factors that would affect implementation of rehabilitation programs and patient-therapist relationships.

Co-treats with other allied health disciplines when emotional and / or behavioral factors interfere with rehabilitation progress.

Provides discharge planning and follow-up services including referrals for patients and families.

Maintains patient documentation per organization requirements.

Performs all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab.

Provide professional presentations as requested (e.g., faculty development, grand rounds, medical school teaching)

Assist with advanced directives, and / or transitions to next level of care.

Serves on various ad hoc and standing committees as requested.

Knowledge, Skills & Abilities Required

Knowledge of patient family care giver support, including the necessary knowledge, experience and clinical skills at a level acquired through the completion of a Master’s degree.

Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) required.

Minimum 1 of year experience in a hospital setting managing a patient family caregiver support caseload. Prior experience in an inpatient and / or outpatient rehabilitation setting is preferred.

Analytical ability necessary to solve complex administrative and clinical problems. Ability to evaluate needs of patient populations and develop programs to meet those needs.

Interpersonal skills necessary to motivate and provide direction as well as represent the SRAlab outside of the Institute to community groups and officials of health and welfare agencies.

Ability to communicate effectively with all levels of management within the Institute.

Working Conditions

Normal office environment with little or no exposure to dust or extreme temperature.

Occasional exposure to infectious diseases when working with patients.
