Job Title : Coordinator / Case Manager for Geriatric Services
Hours : Regular Full-Time (35 hours),
Travel : 70% local travel, Hybrid
Salary : $41,000 - $45,000 Annual

JOB SUMMARY
Under the general direction of the Program Manager, but working collaboratively with the Senior Care Options (SCO) representative(s), the SCO Case Manager plays an integral part of the case management functioning of the agency.
A SCO Case Manager conducts assessments of the SCO client's needs and determines eligibility for any relevant services and resources.
The SCO Case Manager also develops service plans which would address the clients' needs and concerns based on the assessments.
This Position is best suited for one who can work autonomously, is empathetic and can multitask as the roles involves conducting home visits, family / provider meetings, telephonic contact and regular documentations.
Besides interacting with clients and their formal and informal supports, the SCO Case Manager works collaboratively within BSHC as well as with SCO representatives, vendors, potential referral sources and other community agencies.
AN IDEAL CANDIDATE WOULD BE :
- Bachelor's Degree in social work, human services, nursing, psychology, sociology, or related field is preferred.
- SW, LSW and relevant work experience preferred, but not required.
- Bilingual in English / Cantonese / Mandarin
A SUCCESSFUL CASE MANAGER WOULD HAVE :
- Sensitivity to elder issues and commitment to maintaining clients at home with dignity.
- Precise record keeping, excellent oral and written communication skills and a commitment to teamwork is essential.
- Excellent assessment and documentation skills required.

One of 26 Aging Services Access Points (ASAPs) in Massachusetts, Boston Senior Home Care is committed to providing the best assistance possible for those in need.
Although the majority of the people we assist are over age 60, we offer long term services and supports for children and young adults.
We also provide programs and education initiatives for those serving as caregivers and information about maintaining a healthy lifestyle and controlling chronic disease at any age.
