Overseen by At Home Division's office manager, The Palliative Care Clinical Nurse Navigator / Intake Support Specialist provides support to the palliative care and hospice programs.
Intake Functions :
- Ensures timely processing of all palliative care referrals, obtaining orders for patients to receive palliative care services, providing necessary information to insurers as needed, preparing the electronic medical record with all needed information to ensure a smooth transition onto service, and coordinating palliative care visit schedules.
- Supports hospice intake by obtaining verbal orders for admission, reviewing medical records to assist our physicians in determining eligibility, and assisting with medication reconciliation.
Clinical Nurse Navigator functions :
- Will play a pivotal role in supporting patients and their families facing serious illness by providing comprehensive care coordination and guidance throughout their healthcare journey.
- Close collaboration with our nurse practitioners, community healthcare providers, and patients and their families to ensure seamless transitions, optimize symptom management, and enhance the overall quality of life for patients receiving palliative care.
Additional duties to include occasional direct care nursing coverage in times of high-volume or staffing shortage.

Requirements :
- LPN licensure in the State of MA required
- 3-5 years of experience in hospice and / or palliative care required
- Excellent communication, interpersonal, and organizational skills required
- Knowledge of insurance benefits and eligibility required
- The ability to handle complex and confidential information required
- Solid critical thinking skills and attention to detail required
- Excellent written and verbal communication skills to interact with all levels of personnel, both within and outside company required
- Knowledge of the Hospice Conditions of Participation required
- Ability to work with an electronic medical record required
- Knowledge of Microsoft Outlook, Word, and Excel required
- Ability to work collaboratively with administration staff, office staff, and field staff required
Work Hours and Schedule Expectations :
Typical workday hours are Monday through Friday from 9 : 00AM to 3 : 30PM. Employee within this position is not expected to work paid holidays.
