Orthopaedic Clinical Nurse Navigator - 10K Sign on bonus
Connected Health Care is seeking a dedicated and experienced Orthopaedic Clinical Nurse Navigator for a full-time position in Alamogordo, NM.
This role offers a $10,000 sign-on bonus. Join our team to provide expert care and support to orthopaedic patients, improving their outcomes and enhancing their overall experience.
Job Description :
- Provide expertise as a professional RN or LPN with Orthopaedic clinical knowledge.
- Serve as a patient advocate and educator, guiding patients through their care journey.
- Collect, track, trend, and analyze data to improve patient outcomes.
- Promote evidence-based clinical practice within the orthopaedic department.
- Assist in the coordination of care, acting as a contact and resource for patients.
- Collaborate with multidisciplinary teams to ensure comprehensive patient care.
- Maintain accurate and timely documentation of patient interactions and progress.
Education and Certification Requirements :
- Active NM Licensed RN, BSN
- ONC preferred within (2) years of employment
- Two (2) years relevant experience preferred
- Current American Heart Basic Life Support (BLS) certification
