You will work collaboratively with an interdisciplinary team to provide comprehensive care and coverage for older adults, enabling them to live the independent lives they deserve.
$127,700 - $151,100 a year
They offer a robust compensation package for this role that includes cash compensation and other total rewards. Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role.
Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
Key Responsibilities
- Provide direct medical care to PACE participants
- Complete sections of assessments and care planning
- Manage a panel of participants
- Initiate, evaluate, review and change orders for medications and diagnostic studies or treatments with approval
- Assess, initiate and coordinate care on a daily basis under supervision
- Document all participant interactions, assessments, next steps in the EMR within the specified timeframe set by the center
- Make referrals to specialists with approval and request consults as needed
- Perform history and physicals and participant teaching
- Order, interpret and follow up on lab tests and results
- Perform selected procedures
- Recognize and escalate abnormalities as needed
- Liaise with other team members for participants care, including behavioral health and pharmacy
- Evaluate and treat participants where they are located, whether that be in the center, home, virtually, SNF etc
- Facilitate advanced planning discussions with participants
- Document chronic conditions via HCC capture
- Provide on-call coverage as needed
Schedule and Shift Details
40 hours per week, no on-call
