Conduct initial assessments and develop personalized care plans for clients
Administer medications and treatments as prescribed by healthcare providers
Monitor and document clients' progress and report any changes to the healthcare team
Provide skilled nursing care, including wound care, catheterization, and tracheostomy care
Assist with activities of daily living such as bathing, dressing, and grooming
Educate clients and their families on proper care techniques and disease management
Collaborate with interdisciplinary healthcare team to ensure holistic care for clients
Maintain accurate and thorough documentation of all care provided
Qualifications :
Valid nursing license in the state of Kansas
Minimum of (insert number) years of nursing experience, preferably in a home care or long-term care setting
Experience with electronic medical records (EMR) systems
