Develop diagnoses and plan of care within a holistic, trauma-informed, biopsychosocial framework, utilizing a full scope of therapeutic and psychopharmacological skills in the treatment of complex psychiatric conditions.
Includes comprehensive case management, crisis intervention, health promotion, advocacy, interdisciplinary education, and consultation to 12 primary care providers across 6 primary care teams, in collaboration with integrated behavioral health care.

As the Corrections Health Psychiatric Mental Health Nurse Practitioner, you will : Perform comprehensive health assessments and clinical procedures by completing a physical and psychosocial historyDiagnose and treat acute and chronic behavioral health conditions according to scope of practice to include conducting psychiatric evaluations, mental status exams, ordering and interpreting diagnostic tests, devising a course of treatment, discussing alternative treatment, risk and benefits of treatment, prescribing and monitoring medications, providing brief counseling of patients and as needed, family members, and reassessing the patient at appropriate intervals.
Utilizes panel-based information technology tools to prioritize and direct proactive care for patients assigned to provider panels including preventive health screening and chronic disease care management.
Follow-ups on utilization issues (ED, hospital admissions).Documents actions taken, staff assignment of duties, etc.Participate in conducting psychoeducational groups including evaluation and referral of patients.
Provide consultation to primary care providers, nursing staff, health assistants and other behavioral health team members regarding patient behavioral health concerns.
Provide basic care management and linkage services including specialty behavioral health referrals for clients for whom providing behavioral treatment.
Establishes need for specialty and rehabilitative services and makes appropriate referrals.Assesses and counsels patients regarding preventive care needs;
delivers services or makes appropriate referrals.Determines need / schedule for high-risk referrals. Notes referrals and outcomes in documentation for care plan and charges / reimbursement of services.
Attend and participate in all required staff meetings, clinical psychiatric consultation and other ad hoc committees as assigned.
Serve as participant and subject matter expert for mental health services for adults in correctional facilities.Review, evaluate and join committees of internal and external parties on policies and procedures for providing mental health evaluations and care for clients in correctional facilities, taking into account cultural and other needs and factors.
Provide coverage for clinical colleagues in their absence.Completes patient documentation and manages electronic in-basket according to established policies and procedures.
Manages confidential health information as part of chart notes and documentation to set care plans for other providers and staff to provide team approach healthcare delivery.
Utilize therapeutic treatment, and use of examination instruments to assist in providing evident-based psychiatric treatment in the correctional settingsDevelop and modify treatment plans, including further diagnostic studies, therapy, patient education, and identification of parameters for follow-up on psychiatric conditions.
Monitor vital signs, nutritional intake and signs and symptoms of complications to effectively address emergent psychiatric conditionsPassionate about learning and committed to continual improvement.
Develop and maintain courteous and effective working relationships. Minimum Qualifications / Transferable Skills Masters degree in Nursing and one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.
Certification Requirements : Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner (PMHNP) license.National certification from appropriate organization in Psychiatric / Mental Health : American Nursing Credentialing Center (ANCC)Prescriptive Authority must be maintained through the Oregon State Board of Nursing per ORS 678.
