Essential Job Functions
The Home Care Nurse works with minimal supervision to assess, plan, implement, and evaluate patient care. Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, decision making, priority setting, work delegation and work organization.
The Home Care Nurse supports agency policies, procedures, protocols, and standards of nursing practice recognizing each person as an individual.
The Home Care promotes positive patient experiences, demonstrates the agency’s core values, and exemplifies the agency’s philosophy in daily work activities, promotes and maintains effective relationships and communication in all professional endeavors.
Effectively communicates with clinical managers and members of the care delivery team. Provides oversight of LPN and HHA if applicable and completes required supervisory visits.
Demonstrates strong clinical, assessment, and critical thinking skills. Responsible for delivery of competent and quality care in the homes of patients that conforms with agency policies and procedures.
Responsible for coordination of care for home care patients and results in positive quality outcomes for the patients served.
Responsible for providing great patient experiences. Educating on health, wellness, and self -management for the patient.
Serves as an integral link between the patient and access to community-based resources. (DME, supplies, infusion, community resources, SW, etc.
Conducts admission and follow up visits according to the plan of care. Strong computer and mobile device skills.
The Home Care Nurse develops, establishes, implements and evaluates an individualized plan of care for patients that corresponds with physicians’ orders.
Performs assessments of patient's needs, condition, environment, and consults with other health team members and updates the plan of care as necessary.
Maintains ongoing responsibility for assigned case load. Provides teaching to patients, families, and caregivers. Participates and communicates in scheduled case care conferences with the health care team.
Responsible for oversight of patient case load, visit frequencies, orders, insurance authorizations, and overall coordination of care for assigned patient.
Monitoring delivery of services by the other team members, directing care toward the achievement of outcomes. Adheres to productivity standards set forth by the agency.
Schedules patient visits weekly and communicates schedule plan to the scheduling team and communicates any changes to the plan.
Develops, implements, and communicates discharge plan in conjunction with the patient, caregivers, health care team, and physicians.
Proficient and competent in completing assessment and required OASIS documentation. Completes all required documentation timely, per agency policy, procedure and requirements.
Demonstrates an understanding of documentation and assessment to quality outcomes and reported measures. Demonstrates financial stewardship and complies with agency budget.
Uses agency equipment and supplies as required with cost effectiveness in mind.
Attends and participates in staff meetings. Participates and contributes to Quality Assurance and Performance Improvement projects.
Demonstrates behaviors that are in line with Mercy Health mission, values, and standards of performance. Acts as a collaborative team member and promotes teamwork across all disciplines.
Demonstrates knowledge with regulatory requirements and practices as guided by CMS, JCAHO, ODH and other regulatory entities.
Adheres to productivity standards set forth by the agency.
Required Minimum Education : Diploma in Nursing or 2 Year / Associates Degree - Nursing
Preferred Education : 4 year / Bachelors Degree - Nursing
Licensure / Certification Required - RN currently licensed to practice nursing in the State of Ohio. Certified in basic CPR.
Valid driver's license with a driving record that meets BSMH insurability requirements.
