The Nurse-Family Partnership is a program within Adams County Health Department Nursing Division that is a national, evidence-based maternal, infant and early childhood home visitation program which provides fosters long-term success for pregnant and parenting clients, families, and their children and families.

56NFP Nurse Home Visitor PHN II : $72,968.11 - $83,913.33NFP Nurse Home Visitor PHN IThe Nurse Home Visitor PHN I is an entry level position performing professional nursing process activities related to the perinatal period such as assessment, planning and evaluating care and treatment of birthing adults and infants up to two years.

Must have completed an accredited Bachelor of Nursing degree program and has up to two years of nursing practice. The NFP NHV PHN I is learning program level processes and interdepartmental roles.

The position develops therapeutic relationships, provides evidence-based education, support, and resources for the families in the community.

Participates in continuous quality insurance activities, and practice improvement activities such as Practice Council. Recognizes the Public Health tenants including the public health nurse role and functions with responsibility as required by the Public Health Nursing Scope and Standards of Practice.

NFP Nurse Home Visitor PHN IIThe Nurse Home Visitor PHN II is a mid-level position that demonstrates advanced nursing process skills through nursing experience or competency.

Must have more than two years nursing experience, preferably maternal / child health. In addition to the responsibilities noted above, this position requires the NFP NHV PHN II to have additional professional skills and experience in : active engagement in continuous quality improvement,active participant on agency and community committees,competency assessment,creative problem solving,critical thinking,outcomes measurement,practice improvement leadership such as Practice Council Chair / Co-Chair,public health and program model of care champion,teaching, precepting and mentoring, andtraining and on-boarding new PHNs.

Examples of Duties for SuccessClinical ResponsibilitiesPromotes the maintenance of health through teaching and counseling, conducting needs assessment and case management, providing direct client care and / or resource coordination and employing appropriate preventative treatment and health maintenance measures : Develops and maintains therapeutic relationships with a diverse and complex client population, utilizing concepts of reflection and motivational interviewing;

modeling client-centered, strengths-based and solution-focused approaches.Recruits and enrolls a caseload of 25 active clients by 9-12 months post-hire and maintains Colorado caseload targets thereafter, per Colorado Caseload Maintenance GuidelinesContinues to monitor and support client engagement in the program and proactively implements strategies to retain clients in the program.

Ensures and adheres to professional boundaries.Formulates and implements plans for nursing and general health care for individuals and families based upon evaluation of needs and a focus on meeting mutually accepted health goals and objectives.

Delivers nursing services through home visits, case management and community outreach programs.Completes comprehensive and holistic nursing assessments, including model specific assessments such as Dyadic Assessment of Naturalistic Caregiver-Child Experiences (DANCE), Intimate Partner Violence clinical pathway ASQs, PHQ-9, and GAD-7.

Formulates nursing diagnosis based upon nursing assessments, STAR Framework and client goals, and develops a plan of care with the client.

Assist clients in establishing realistic and achievable goals and measurable outcomes for promoting their health, caregiving abilities and life course development.

Plans home visits in accordance with client goals and NFP outcomes.Provides education and in support in assisting women and their families in attaining their targeted goals and educational needs.

Assists clients in finding resources to meet needs and makes referrals for services as appropriate.Consults and collaborates with other professionals involved in providing services to women and families;

provides timely and coordinated case management services with a variety of health, behavioral health and social service providers.

Documents health history, assessments, goals nd objectives, and care provided in compliance with department and program policy, procedures and protocols.

Completes NFP clinical documentation thoroughly and in a timely manner.Completes NFP data collection forms and associated assessments, per NFP required timelines and with a high degree of accuracy.

As required, completes Medicaid billing documentation per agency and state expectations and ensures that all billable services are submitted within timely filing guidelines.

Participates in review and analysis of the ETP reports to identify achievements and areas for improvement. Participates in quality improvement efforts.

Maintains confidentiality of information.Reflective Practice ResponsibilitiesAttends and participates in NFP required weekly reflective / clinical supervision sessions with NFP supervisor to facilitate individualized care planning for clients to facilitate optimal client outcomes and mitigate nurse home visitor secondary vicarious traumaSchedules, prepares for and completes three join home visits with NFP Nurse Supervisor each year with a regular cadence.

Utilizes principles of reflection to debrief on visit experiences.Assumes responsibility for ongoing learning in relation to program implementation and actively engages in professional development to meet all NFP Nurse home Visitor competency requirementsAttends and participates in team based reflective practice by contributing to team meeting and case conferences discussions at both eh small team nd large team levels.

Collaboration and Community Engagement : Team Development and CollaborationAssists in creating positive, inclusive work environment that promotes productivity, mentoring, teamwork and cooperation at the small team, large team and programmatic levels.

Participates in individual, team and programmatic review of NFP data and development and implementation of quality improvement strategies.

Represents ACHD as a health resource in the community. Interprets public health programs and policies to individuals, families, community groups, schools, physicians, and other collateral community agencies and service providers.

In the course of traveling to and working in other locations, may experience exposure to noise, dust, solvents, fumes, smoke or gases, slippery / uneven walking surfaces, sun, road hazards, traffic considerations, motor vehicle accidents, and exposure to inclement weather such as driving in snow, ice, etc.
