Optum is seeking a Nurse Practitioner or Physician Assistant, HouseCalls Statewide Traveler to join our HouseCalls team in Eastern NY Region. Optum is a clinician-led care organization, that is creating a seamless health journey for patients across the care continuum.   The locations of travel will be :  Albany, Clinton, Columbia, Delaware, Essex, Franklin, Fulton, Greene, Hamilton, Herkimer, Jefferson, Lewis, Montgomery, Oneida, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, St. Lawrence, Ulster, Warren, Washington.

As a member of the broader Home and Community Care team, you’ll provide annual clinical assessments to patients in the comfort of their homes. This important preventive care helps identify and reduce health risks for patients, in addition to coordinating appropriate follow-up care to improve health and well-being.

In this role you will have the ability to achieve work life balance. Flexible scheduling offered where providers can flex their time over the 7-day work week. No on-call, no weekends and no holidays required.

This role requires travel across the entire state of

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $89,900 to $160,600 annually based on full-time employment. We comply with all minimum wage laws as applicable.
