We are currently seeking a highly skilled and compassionate Physical Therapist to join our esteemed team at Always Best Care Senior Services - Oahu in the picturesque city of Honolulu, Hawaii.

Working within the comfort of their own homes, in accordance with physician orders, you will provide exceptional and personalized care.
This intermittent position offers flexibility, allowing you to contribute on a schedule tailored to your needs and preferences.
Responsibilities :
- Conduct comprehensive assessments to determine the unique needs of patients, both initially and on an ongoing basis.
- Collaborate closely with physicians to develop and revise personalized treatment plans based on patient progress.
- Assist in the development and implementation of interdisciplinary patient care plans, specifically related to Physical Therapy.
- Work closely with physicians to administer diagnostic and prognostic tests to evaluate patients' muscle, nerve, joint, and functional abilities.
- Utilize a range of physical therapy techniques, including exercise, massage, heat, water, light, and electricity, to alleviate pain, enhance function, and optimize performance.
- Consult and educate staff members on the established treatment plan.
- Instruct and guide patients and their families in the physical therapy program.
- Educate patients on the proper use of wheelchairs, braces, crutches, canes, and prosthetic and orthotic devices.
- Provide necessary equipment to support the treatment plan.
- Coordinate outpatient services for procedures that cannot be administered at home.
- Maintain accurate and timely records of evaluations, treatments, and patients' response to therapy intervention.
- Prepare comprehensive assessments, evaluations, and summaries of patients' conditions.
- Supervise Physical Therapy Assistants and paraprofessionals, adhering to agency policies.
- Report any changes in patients' condition or reaction to treatment to attending physicians.
- Effectively communicate with physicians and the team to ensure comprehensive patient care.
- Record detailed clinical and progress notes, incorporating them into patients' records in accordance with agency policies.
- Participate actively in case conferences, physical therapy staff meetings, agency activities, and in-service programs as required.
- Oversee the work of Home Health Aides and Physical Therapy Assistants when physical therapy is the sole discipline involved in patient care.
- Contribute to discharge planning activities and complete Physical Therapy Discharge Summaries.
Requirements :
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A., in collaboration with the American Physical Therapy Association.
- Valid license to practice Physical Therapy in the state of Hawaii.
- Preferred : Minimum of one year of institutional / home health experience.
- Criminal background check required.
- Current CPR certification.
