General Description
The Licensed Practical Nurse is a valuable member of our Behavioral Health team who is responsible for providing nursing care to people with mental health and / or substance use diagnoses in a 24-hour behavioral health residential program, including intake assessments, administering medications, and ongoing medical care, under direction of RN and medical provider.
LPN is responsible for direct care of clients, including crisis intervention and triage on admission, participating in developing treatment plans with clients, and providing emergency assessment for possible hospitalizations and for supporting medical and psychiatric providers on the unit.
Schedule : 37.5 hours a week
Essential Functions
- Provides triage and medical screening for admissions to the program, under direction of RN and medical provider.
- Providesprofessionalnursingcare,includingadministeringmedications,accordingtoprovider'sordersinconformancewithrecognizednursingtechniquesandprocedures,establishedstandardsandadministrativepoliciesofthedepartment.
- Recognizesandinterpretssymptomsofclients'conditions,reportstoappropriatepersonnelandassistswithremedialmeasuresforadversedevelopments.
- Conducting point of care testing.
- Monitorsthecollectionofurinespecimensaccordingtostandardprecautions.
- Maintainsclinicalchartsincludingdocumentationofobservations,medicationstreatments;reportsontheconditionofclients;observesandcorrects,ifpossible,environmentalfactorsdealingwithcomfortandsafety.
- Participatesinstaffmeetingsandprovidesinputregardingtheclient'sphysicalcondition.
- Isresponsibleforarrangingclinicappointments,chestx-rays,laboratoryworkandothermedicalappointments.
- Makesarrangementsformedicaltransferofpatientstoothertreatmentfacilities,asnecessary.
- Respondstoinquiries,givesappropriateinformationandreferrals,anddocuments,ifnecessary.
- Involvedinqualityimprovementactivitiesonanongoingbasis.
- Takesapersonalstakeinthesuccessoftheprogram.
- Focusesonclientempowermentinprovidingtreatment,seekingtoassistclientstobuildonstrengthsinaddressingtreatmentneeds.
- Facilitateswithclientsandcollaborateswiththetreatmentteamtodevelop,reviewandupdatetreatmentgoalsandreferralsbothinternallyandexternally.
- Organizesandparticipatesintreatment-relatedclinicalconferencesandstaffingsonassignedclients.
- Provides education and direction to clients, family members and / or significant others.
- Takesresponsibilityfortraininginanddocumentingclinicalchartrequirementstomeetprogramandstatestandards.
- Mayassistintheorientationandtrainingofnewstaff.
- Participatesinstaffmeetings,in-servicetrainings,andseminarstofacilitateprofessionalgrowthand maintainlicensureifrequired.
- Mustmeetminimumproductivityexpectations.
- Maintainsanawarenessofandprofessionalinvolvement / collaborationwithcommunityresources.
- Assists in facilitating transportation of clients.
Knowledge Skills Abilities and Education Required
- One year experience as an LPN required; one year experience in a behavioral health setting preferred.
- Illinois Licensure as a licensed practical nurse (LPN).
