Conducts and completes comprehensive psychosocial assessments
Conducts and completes mental health and suicide assessment screenings
Facilitates clinical counseling and emotional support by utilizing and enacting clinical therapy models
Participates in and conducts discharge planning in collaboration with internal and external facilities / stakeholders as needed
Refers patient, client, and / or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education
Provides effective care and case management per plans of care
Participates in required supervision for staffing of cases and professional growth towards licensure
Assesses the capacity of the home caregiver to cope with the participant / patient's needs in the development of the plan of care
Engages clients in exploration of goals for care. Promotes and empowers clients to be an active participant in healthcare decisions
Facilitates family meetings as needed
Maintains knowledge of current regulatory, accreditation, and third-party payer guidelines as well as state and federal regulations that impact client populations

Required Education : Masters Degree in Social Work from a Council on Social Work Education (CSWE)-accredited institution within 6 months of hire.
Required Experience : Minimum of three (3) years in case work or counseling in a healthcare setting or social work delivery system
- Required Certifications and Licensures : License eligible for the Licensed Clinical Social Worker (LCSW). All caregivers will be required to obtain LCSW license within three (3) to six (6) years of hire date;
- a specific agreement will be placed in the caregiver's HR file. Per the Virginia State Board of Social Work, any person pursuing a clinical social work license will need to meet the following requirements to take the examination;
- these requirements ensure license eligibility : Successfully graduate with a Masters Degree in Social Work (MSW) with a clinical course of study from a CSWE-accredited program;
- Apply for Supervision Approval (Registration of Supervision); Gain Post-MSW Degree supervised experience in the delivery of clinical social work services;
- Apply for LCSW Licensure by Examination; Pass the ASWB Clinical level examination For Clinical Social Worker positions with Home Health and PACE, driving is an essential function of this position;
Valid Virginia Driver's License with a Department of Motor Vehicles (DMV) record in good standing, per Centra guidelines.
When driving a Centra-owned vehicle, Centra is the primary insurer. When driving a privately-owned vehicle for work, personal insurance attached to the vehicle is the primary insurance;
Centra insurance will supplement for catastrophic events. Centra will review DMV records annually and verify personal insurance as needed
