Job Summary :
Provides holistic, quality and cost-effective nursing care for patients / families with diabetes within the institution, service or other primary area of practice with an awareness of the scope of care.
Applies the nursing process within the context of the organization’s Framework for Professional Practice. Functions as clinical expert in diabetes.
Assists with staff education and development. Assists with development of policies and protocols for care of the patient with diabetes.
Delegates and supervises work appropriately. Reports to the Diabetes LifeCare Manager.
Qualifications
- Current Connecticut RN License
- Graduate of an accredited nursing school. ASN Required, BSN preferred.
- BLS from the American Heart Association
- CDE (Certified Diabetes Educator) preferred, but not required. Must obtain certification within 2 years of hire.
- Bilingual in English and Spanish a plus
- The following diabetes / endocrine experience is preferred but not limited to :
- Type 1 DM
- Type 2 DM
- Gestational DM
- Insulin pumps and continuous glucose monitors
- Diabetes / Endocrine direct (individual and group) patient education and management a plus. Some evening class commitment required.
- Electronic Medical Records experience preferred.
- Knowledge of ADA Clinical Practice Guidelines helpful.
