A company is looking for a Care Connector (B) to support pregnant and postpartum members.
Key Responsibilities
Facilitate and coordinate care for maternity members, serving as a single point of contact
Assess maternity members to determine care coordination needs and address issues / concerns
Coordinate care and services with members, providers, and Community Based Organizations
Required Qualifications
Bilingual in Spanish
High School diploma or GED
Minimum of 2 years of previous work experience, preferably in a health care setting
Managed care experience preferred
Experience with medical terminology / documentation is a plus
