Job Description :

Collaborative Care Social Workers (Population Health) are placed in primary care and specialty medical clinics such as : adult primary care, pediatric primary care, womens health services, virology, etc.

working within the Collaborative Care model, which integrates behavioral health into ambulatory care medical settings. The goal of Collaborative Care is to meet patients where they are;

providing access to behavioral health care within a patient’s healthcare setting. Collaborative Care Social Workers (Population Health) work as part of an interdisciplinary care team providing treatment to patients experiencing common mental health challenges such as depression and anxiety, and other mental health conditions.

Collaborative Care Social Workers (Population Health) will receive training in the Collaborative Care treatment model and other topics related to integrated behavioral health care in medical settings.

Collaborative Care Social Workers (Population Health) will have the opportunity to participate in trainings related to evidence based practices utilized in Collaborative Care such as : Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical Behavior Therapy, Interpersonal Psychotherapy, Problem Solving Treatment, etc.

Placement opportunities may be available at NYC H+H facilities in all 5 boroughs.

Clinicians will obtain clinical hours applicable towards LCSW licensure under structured individual and group supervision.

General tasks and responsibilities will include :

Under supervision, is trained in and performs the following typical tasks :

  • Screens and assesses patients for mental health and substance use disorders. Facilitates patient engagement and follow-up care.
  • Conducts initial intake assessments and treatment plans for patients with behavioral health needs.
  • Engages clients in short-term individual (in person or over the phone) and / or group counseling sessions using evidence-based interventions such as motivational interviewing, problem solving therapy, cognitive behavioral therapy, and more
  • Supports patients throughout their enrollment in Collaborative Care treatment (assessment, treatment, relapse prevention planning, discharge, etc).
  • Facilitates referrals for clients to concrete social services and community-based agencies as required and ensures appropriate and timely service delivery to clients, advocating and serving as a liaison for patients with community agencies where needed.
  • Facilitate patients’ referral to higher levels of care where relevant upon referral to or discharge from Collaborative Care.
  • Supports psychotropic medication management, as prescribed by medical providers, focusing on treatment adherence monitoring, side effects and effectiveness of treatment (when applicable).
  • Adheres to record-keeping in the EHR and other required systems, reports, and related correspondence and / or paperwork as per Health + Hospitals protocols.
  • Complies with necessary documentation, including utilization of clinical patient symptom rating scales, to track patient outcomes and meet regulatory requirements regarding Collaborative Care billing and reporting.
  • Makes recommendations and facilitates changes to care plans based upon documented progress of clients toward goals, and collaborates with the care team in adjustment of care plans.
  • Identifies, evaluates and designs proper safety plans for victims of crimes / abuse, i.e : child abuse, elder abuse, domestic violence, rape and sexual assault.

Discusses these patients and safety plans with supervisor.

  • Identifies and develops working relationships with appropriate community-based, culturally and linguistically competent agencies that offer services within clients’ communities.
  • Participates in weekly individual and bi-weekly group supervision and keeps supervisor apprised of progress with clients, client needs and problems during supervisory sessions.
  • Appropriately escalates emergencies to supervisory and administrative staff, including cases of patient danger to themselves or others, and reports suspected cases of abuse or maltreatment following H+H protocols.
  • Attends and participates in regular interdisciplinary Collaborative Care case conferences.
  • Attends in-service training sessions, staff meetings and other mandatory trainings, workshops and events, as requested by H+H.
  • Works within facilities to provide concrete social services to patients as required.
  • Represent Collaborative Care program in designated clinical setting to ensure all staff understands Collaborative Care program and referral criteria and process.
  • Performs other related duties, as directed and with required supervision.
  • Section 424-a of the New York Social Services Law requires an authorized facility to inquire whether a candidate for employment with child-care responsibilities has been the subject of a child abuse and maltreatment report.

The facility has the discretion to assign a candidate who has been the subject of child abuse and maltreatment report to a position with no child-care responsibilities.

Minimum Qualifications :

Valid license as an LMSW issued by the NYSED

Department Preferences

  • Knowledge of child and / or adult development, as well as knowledge of working with individuals, groups and families.
  • Familiarity with social services, educational, vocational, health, and families
  • Spanish or other secondary language skills in addition to English
