Who are we looking for?

A compassionate, dedicated RN Case Manager who can bring comfort and care to our patients in their homes, assisted living, or skilled nursing facilities.

We want someone who is confident in their skills, able to navigate the complex world of hospice and palliative care, and committed to providing consistently exceptional customer service.

Qualifications :

• Current GA Registered Nurse (RN) licensure required.
• Dependable transportation and valid state driver's license.
• At least one year of hospice or home health experience.
• Strong communication and observation skills, with a focus on providing compassionate care.
• Understanding of hospice principles and philosophy of care.
• Experience working with interdisciplinary teams.

Your Role as an RN Case Manager :

• Provide top-notch bedside care to our patients and their families.
• Educate patients and families about the disease process, self-care techniques, end-of-life care, nutrition and dietary needs.
• Collaborate with the hospice team to ensure comprehensive care.
• Manage symptoms and handle the needs of patients at the end of life.
• Maintain accurate and timely clinical records, including assessments, physician orders, progress notes, and care plans.
