Job Summary : The Lead LVN will oversee daily operations and provide direct patient care in a clinic setting. This role includes supervising other LVNs, coordinating patient care, and ensuring high standards of clinical practice under the direction of an RN or physician.
Key Responsibilities :
Supervise and support LVN staff, ensuring adherence to clinical protocols and procedures.
Coordinate patient care activities, including scheduling and workflow management.
Administer medications and treatments as prescribed.
Conduct patient assessments and assist with diagnostic tests and clinical procedures.
Provide patient education and support regarding treatment plans and health maintenance.
Maintain accurate and timely medical records.
Ensure a clean, safe, and efficient clinical environment.
Assist in developing and implementing clinic policies and procedures.
Address patient concerns and ensure high levels of patient satisfaction.
Maintain clinic supplies and coordinate nursing department projects.
Act as a liaison between LVNs and Nursing Management to facilitate effective communication and collaboration.
Train and mentor LVN staff to ensure competency and professional development.
Practice nursing care in compliance with HIPAA standards
Willnes to float through out different sites as needed.
Assist in emergency situations as needed.
Perform other duties as assigned.
Required Qualifications :
Graduate of an accredited LVN program.
Current California LVN license in good standing.
Minimum 3 years of experience as an LVN working with geriatric or frail populations.
Valid CA Drivers license
BLS Certification from the American Heart Association
Bilingual in Spanish, Cantonese, or any other language is highly preferred
Proficiency in electronic health records (EHR) systems.

Population : Geriatric population
Monday - Friday NO WEEKENDS
