Job Description
The Licensed Vocational Nurse (LVN) is a direct-care provider who has the technical proficiency and observation skills necessary for initiating, performing, and completing assigned duties in delivering comprehensive practical nursing care to variable patient populations.
A Clinovators LVN provides skilled nursing care and prescribed medical treatment to Veterans in compliance with the Nurse Practice Act in the State of Texas.
The LVN works under the direction of the supervising Provider or Physician.
Key Duties and Responsibilities
- LPN / LVN, you are responsible the provision of covered services to enrolled and unassigned patients presenting for care at the site
- Providing evaluation and care consistent with licensure, certification, and functional statement with elements of practice, to patients assigned PACTs
- Collaborating with PACT staff to develop comprehensive health care plans and care management plans for patients assigned to patient panels
- Managing clinic workflow, ensuring patients are placed in examination rooms in a timely manner, and providing direction to patients as they move through the clinic environment.
- Duties include but are not limited to the ability to perform a variety of specialized clinical support skills, ability to perform basic patient care service, have knowledge of medical terminology, demonstrate skills in interpersonal communication, demonstrate knowledge of aseptic technique and infection control and knowledge of patient confidentiality, policies, and procedures.
- Shall assist all health care providers in performing patient care services and duties pertaining to the effective and efficient delivery of patient centered care in all clinical areas.
- Serve as back-up for telehealth services.
Knowledge & Skills
- Utilizes judgement in selecting the appropriate order and sequence of procedures and treatments.
- Utilizes judgment in selecting the most appropriate application of procedures and guidelines.
- Initiates and participates in preventative health screening and instructions for ambulatory care patients.
- Observes, identifies and responds to patients' needs for care, including medication and equipment-assisted care, and recognizes deviations from normal.
- Performs venipuncture and inserts peripheral IVs, upon completion of IV class or certification and competency where indicated.
Required Qualifications :
- U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- Education or Training : Graduate of a school of practical or vocational nursing approved by the appropriate State Agency and / or accredited by the Accreditation Commission for Education in Nursing (ACEN);
OR Received health care education / training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualified for full LPN / LVN licensure.
