Shift : Day 5x8-Hour (8 : 00 AM - 5 : 00 PM) with alternating weekends every 3-4 weeks
Guaranteed Hours : Not guaranteed, but additional hours may be available through nearby clinics if needed
Experience : Minimum 1 full year of Adult Home Health or Hospice experience required
License : Must have a valid license in hand
Certifications : CPR and BLS required
Must-Have :
- Proficiency in HomeCare HomeBase (HCHB) and OASIS, must have used within the last year
- Reliable transportation, current driver’s license, and agency-required liability insurance
- Willingness to travel up to 75 miles from the assigned care center
- Open to on-call rotations
Job Description :
- Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures
- Utilize the nursing process to deliver competent care
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses
- Conduct home visits and attend periodic office meetings
- Manage an average of 5-8 patients per day with different point values assigned for each type of visit and activity
- Handle cases such as Start of Care, Wound Care, and Follow-up Visits
- Participate in weekend rotations and on-call duties every 3-4 weeks, as required
- Maintain proficiency in using the EMR systems : HCHB and Pointcare
