LVN Case Manager II - Every Woman Counts

The Licensed Vocational Nurse, Case Manager Every Woman Counts EWC (formerly known as Cancer Detection Program), works within a multi-disciplinary health care team in the delivery of comprehensive services for patients enrolled in the Every Women Counts.

Acting as the primary contact person for the patient and their family, the CDP Case Manager assists the health care team in the development and implementation of a health care plan tailored to the needs of the patient and the patient’s family, and following the Every Woman Counts guidelines, to promote continuity of care, improve referrals, and ultimately reduce the rate and severity of breast and cervical cancer.

Reports to : Program Manager of Women’s Health Departments at assigned site : Day to day operations. Corporate Women’s Health Program Manager, Public Health for Programmatic Responsibilities.

Works closely with the Director of Nursing for clinical supervision.

Qualifications :

1. Must be a high school graduate or have a General Education Development (GED) Certificate equivalency.

2. Satisfactory completion of a state approved program in vocational nursing, plus current license to practice in the State of California.

3. Bilingual in English and Spanish required, with excellent written and oral communication skills.

4. Possess the ability to work independently and as team member.

5. Current, valid CA Driver’s License, valid vehicle liability insurance, and an available vehicle needed.

6. Be willing to travel amongst clinic to provide case management.

7. Cultural competency required.

8. Possess excellent organizational and problem solving skills and have the ability to prioritize.

9. Experience with basic computer skills including word processing and data entry.

10.Process the ability to perform a variety of tasks after training.

11.Must maintain a current Cardio-Pulmonary Resuscitation (CPR) certification, in compliance with corporate CPR policy.

LVN I

A. While an entry level position, experience in practical nursing desirable, preferably in an ambulatory patient facility.

LVN II

B.Must have graduated from an LVN program and have at least one (1) year of experience in an ambulatory care facility with patient education expertise preferred and have worked in one or more of the following areas :

a.Pediatrics CHDP (Children’s Health Detection Program)

b.Family Planning, Title X program

c.Prenatal or Perinatal CPSP

d.Diabetes

e.Adult Care, Chronic Disease

C.Experience must have been as a Licensed Vocational or Technical Nurse within a United States Licensing Jurisdiction

LVN III

D.Must have graduated from an LVN program and have at least three (3) years of experience as an LVN with one year in an ambulatory care facility with patient education expertise required and knowledge in one or more of the following areas :

a.Pediatrics CHDP

b.Family Planning, Title X program

c.Prenatal or Perinatal CPSP

d.Diabetes

e.Adult Care, Chronic Disease
