Infusion Nurse
Summary
The Infusion RN is responsible for coordination and facilitation of home infusion therapy along with coordination and provision of infusion therapy within the Ambulatory Infusion Suite.
The Infusion RN provides clinical and operational support to patients and caregivers throughout the duration of the home or Ambulatory Infusion Suite infusion therapy, with extensive, ongoing interaction with patients, staff, providers, referral sources, and community agencies.
Additionally, the Infusion RN orients and acts as preceptor for new staff. There is on-call responsibility with this role.
Driving may be required for this position.

Essential Job Knowledge
- Assesses, plans, implements, and problem-solves for patient referrals for infusion therapy either in the home or Ambulatory Infusion Suite.
- Performs patient intake and scheduling duties.
- Monitors patient progress toward admission to service collaborating with providers, referral sources, community agencies, and organization personnel to ensure safe plan of care and establish mutual goals.
- Develops and revises infusion nursing care plans.
- Collaborates effectively with the interdisciplinary team regularly and in a timely fashion.
- Documents in electronic medical record per organizational policies and procedures.
- Provides direct patient care, including, but not limited to, medication administration via multiple routes (e.g., oral, intravenous, subcutaneous, intramuscular), venous access device use and site care, patient and / or caregiver infusion therapy education, laboratory blood draws.
- Recognizes and responds to signs and symptoms which indicate change in the physical or psychosocial condition of patients.
- Initiates interventions and performs effectively in clinical emergency situations.
- Performs ancillary tasks including, but not limited to, disinfection and cleaning of patient space and equipment, supply inventory.
- Makes visits to external health care organizations to facilitate discharges, as appropriate.
- Serves as a resource / consultant to on-service patients, home health agencies, and other external stakeholders through relationship building and providing home infusion-related education and problem solving.
- Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
- Infection Control : Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
- Quality : Focuses on improving processes and champions efficiency in daily work activities.
- Safety : Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate.

Licenses
- Valid driver’s license in the State of Wisconsin
- State of Wisconsin professional licensure as a Registered Nurse
Education
- Associate or Bachelor’s degree in Nursing. Bachelor’s degree preferred
- Minimum of 2 years of recent nursing experience.
Miscellaneous
- Discharge planning, care coordination, infusion or home health experience preferred.
- Peripheral IV placement skills preferred.
- CPR / BLS Certified
- Excellent organizational, interpersonal, communication, and customer service skills require
