TITLE : Case Manager Location : Buckeye Lake, OH Here is an exciting opportunity to join a top-ranked hospital as a Case Manager.

Qualifications : Must hold a valid license in the state of Ohio. Requires a Bachelor's degree (BSW) or Master's degree (MSW) in Social Work.
Certification as an LSW, LISW, LPC, or LPCC is expected. Prior experience of at least 1 year is preferred. Hours and compensation : The position offers full-time day shifts with a rotational weekend schedule.
The salary range for BSW is $51K-$70K and for MSW is $56K-$78K, based on experience. A comprehensive benefits package is also provided.
Responsibilities : Provide professional social work services, including problem assessment and resolution, discharge planning, and supportive counseling.
Offer crisis interventions and facilitate therapeutic, support, and education groups to help patients develop coping strategies during times of illness or disability.
Represent The HealthCare System on community boards / task forces as needed. Stay updated on relevant changes in laws affecting social work practice.
Adhere to the ethical codes set by the State of Ohio Counselor and Social Work Board. Additional information : This position reports to a highly skilled and compassionate manager who has high expectations for the team.
