SUMMARY : Works under the general supervision of the Nurse Team Lead and the Director of Transitional Rehab Center. Assists individuals with a disability and / or acute / chronic illness to attain and maintain maximum function.
ESSENTIAL JOB FUNCTION : Duties and Responsibilities
- Possesses or has the ability to attain specialized knowledge and clinical skills necessary to provide care for people with physical disability and acute / chronic illness.
- Performs hands-on nursing care through applying the nursing process.
- Coordinates and delivers disease process education for patients and families.
- Collaborates with other member of the interdisciplinary rehabilitation team to facilitate achievement of patient centered goals.
- Advocates for policies, services and interventions that promote the quality of life for individuals with disabilities and participates in activities that will positively influence the community’s awareness of disabilities and the role of rehabilitation
- Promotes safety by obeying safety rules of St. John’s United. Identifies, reports and / or corrects obvious safety hazards and attends all mandatory safety in-services.

ESSENTIAL JOB FUNCTION : Employee Responsibilities
- Follows policy and procedures on annual infectious disease testing and vaccinations
- Attends all mandatory in-services as assigned.
- Attain and maintain appropriate license to practice in the State of Montana
Requirements
Minimum Requirements : Licensed as a Registered Nurse in the State of Montana. Minimum of a Bachelor’s degree in nursing is preferred.
Willing to obtain certification in rehabilitation nursing within the first year of employment. Must be CPR certified. Three years’ experience that is directly related to the duties and responsibilities specified is preferred.
Knowledge, Skills, and Abilities :
- Ability to function independently in performing assigned duties.
- Must be able to remain calm under stressful and urgent situations.
- Respond to residents needs in a timely and professional manner, recognizing the patient’s rights.
- Performs job tasks following established procedures and reports problems or concerns to supervisor. Must be able to communicate effectively across disciplines and with patients and family member.
- Thorough knowledge of the nursing process and the ability to apply it to the practice setting.
PHYSICAL DEMANDS :
Requires moderate physical work more than 70% of the time, including lifting up to 25 pounds regularly and up to 50 pounds occasionally, standing, pushing, pulling, lifting, walking, and bending for prolonged periods of time.
WORKING CONDITIONS :
- Work involves some exposure to dirt, dust, fumes, unpleasant odors, and / or loud noises.
- Work involves some exposure to hazards, bloodborne pathogens, or physical risks, which require following basic safety precautions.
- Normal health care environment.
- Work involves exposure to plants and animals within the environment.
