Job Description : The LPN or RN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching / visit duties to ensure the highest quality of care.

Ensures that delivery of nursing services is compliant with company / state policies. Available resource person to other health team members in the identification and fulfillment of patient needs.
Maintains liaison with the community’s health-related resources. Qualifications, Knowledge, Skills, and Abilities : Graduate of approved School of Professional Nursing - or - Current state Licensed Practical or Registered Nurse license.
Pediatric nursing experience preferred.
