Description / Other Details :

• Candidate CVs need to have the month & date on their work experience with all gaps in employment accounted for.
• Includes some Chronic Pain Management
• Chart closure within 72 hours
• Schedule : 18 PPD, minimum see 18 PPD, Monday-Friday 8-5
• Mix of pediatrics / adults (Will consider IM only)
• A decision is typically made within 7-14 days
• Accepting DO
• BE will be considered if truly BE (5 years or less since residency)
• Active DEA needed before start
• Credentialing takes 30-90 days (six weeks is common if providers submit app. / supporting documents timely)
• Required immunizations for credentialing : 2022-2023 Flu Vaccine, TB Status / X-Ray Results (results must be within the 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), COVID19 vaccination, and booster (no longer required).
• HCO & Vendor agree to a 30-day notice for ending all assignments.
