REGISTERED NURSE

This position will assist our individuals in maintain ing and sustain ing great physical and emotional health. This position requires an active RN license.

Essential Functions : Coordinate care with primary care physicians, specialists, pharmacies and other medical services and provide direction of care to staff / individualsWork with multi-disciplinary team to develop participants plan of care and coordination of servicesDirect managers and staff to prepare paperwork and oversee individual’s appointments including any follow ups neededMonitor and the ordering and administration of medications to ensure safety and compliance including medication and treatment communication c hartsParticipate in hospital discharge planning, hospice intake and VNA supports within the communityVerify healthcare b inders are kept organized and current and e nsure healthcare r ecords are entered in HCSIS and kept up to dateFacilitate health education and teaching to direct care staff and participate in other trainings / meetings required within departmentPeriodically visit all assigned residential sites to review medications and talk with staff about any issues or educational needsConduct medical audits to ensure compliance, safety and wellbeing of all involvedTravel to surrounding areas including Malden, Reading, Saugus, Danvers, Stoneham, Beverly and Peabody (and other areas as sites open / change)QualificationsMA RN License (required)Knowledge of case management systems, community service and delivery systems for DD population (required)Attention to detail and excellent time management skills (required)Strong clinical communication skills are essential (required)Minimum of 1-year experience working with DD adults in long term car or human service agency (preferred)Experience with staff training and development (preferred)Language SkillsAbility to verbally communicateAbility to read, analyze, interpret, and write documents.

Ability to respond and navigate common inquiries or complaints from regulatory agencies, or family members.
