We are currently looking for a Bilingual (Mandarin or Cantonese and English speaking) Hospice Medical Social Worker (MSW) Per Diem
Assurance Hospice is a local Arcadia, CA based hospice agency serving the communities and surrounding areas of LA County, San Gabriel Valley and Inland Empire .

General Responsibilities include but not limited to :
- Assesses the psychosocial status of patients / families / caregivers related to the patient's terminal illness and environment.
- Carries out psychosocial evaluations and plans intervention based on evaluation findings.
- Counsels patient / family / caregivers as needed in relationship to stress, and other identified coping difficulties.
- Serves as liaison between patients / families / caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support patient care.
- Participates in the development of the Plan of Care.
- Assists physician and other team members in understanding significant social and emotional factors related to health problems and death / dying issues.
- Participates in discharge planning when needed. Assists patient / family / caregiver with securing durable power of attorney and with funeral arrangements, as needed.
Background Required :
- A Master's degree in Social Work is required.
- Minimum of one year's experience in health care. Prefer experience with geriatrics, home health, hospice, or end-of-life care.
- Understands hospice philosophy and issues of death / dying.
- Demonstrates good verbal and written communication, and organization skills.
- Able to travel to patient locations.
- Bilingual (Mandarin / Cantonese) preferred.
